A patient presented with complaints of oligomenorrhea, hirsutism, and acne. They also find it difficult to achieve weight loss despite regular exercise and diet.

Physical exam revealed obesity (BMI 30), thinning scalp hair, acne vulgaris, and facial hirsutism without signs of virilization (e.g., clitoromegaly) or muscle weakness.  Transvaginal ultrasound findings include enlarged ovaries with multiple small cysts. Serum LH level is 40 IU/L (Normal range: 10-20 IU/L), serum FSH is 10 (Normal range: 4-8 IU/L) and total testosterone (measured by tandem mass spectrometry) is 150 ng/dL (15-70 ng/dL).
