A young adult patient with sickle cell disease is admitted for management of intractable pain from vaso-occlusive crisis. Of note, they underwent red cell exchange transfusion at an outside facility several days prior to presentation. Initial type and screen had a negative antibody detection test for RBC alloantibodies. The patient had a notable decrease in hemoglobin over their hospital stay. A direct antiglobulin test (DAT) was performed.
